    A study of Nihon Buyô (The Japanese Classical Dance) started by Tsubouchi Shôyô (1859–1935). He published Shin gakugeki ron and Shinkyoku Urashima in 1904. Then he established Nihon Buyô as a new kind of dance, in both theoretically and practically. Tsubouchi Shikô (1887–1986) Shôyô\u27s nephew, engaged in Shôyô\u27s study of Buyô from his childhood, and also contributed to develop Nihon Buyô. However, several studies have been made on Tsubouchi Shikô were focused on his studies of Shôyô, theater works in the Takarazuka, and studies of Shakespeare, untill now. Therefore, little attention has been given to his achievement in the field of Nihon Buyô. The purpose of this paper is to examine the point about Nihon Buyô by Tsubouchi Shikô, and to position him into the history of Nihon Buyô studies. First, I describe Tsubouchi Shikô\u27s brief history, and clarify his conventional evaluation, by arranging preceding studies. Then, I follow his whole life with relation to Nihon Buyô. Further, I analyze his essays on Nihon Buyô written in the magazine Geinô. Tsubouchi Shikô probed “the essence of Buyô (dance)” by exploring the origin of dance. And he objected to regard Kabuki Buyô as a model of Nihon Buyô. He expected artists to create new work, based on “the essence of Buyô”, that is, more free from Kabuki Buyô

    Efficacy of Corrected Rapid Turnover Protein Increment Index (CRII) for Early Detection of Improvement of Nutrition Status in Patients with Malnutrition

    Serum prealbumin level is useful for assessment of changes in nutritional status but it is markedly affected by the inflammation. In this study, we examined the efficacy of the corrected rapid turnover protein increment index (CRII) for prealbumin, which is calculated as [prealbumin level/C-reactive protein (CRP) level on the assessment day]/[prealbumin level/CRP level on the day of starting nutritional care], for prediction of improvement of nutritional status in patients with malnutrition. The subjects were 50 hospitalized patients with low albuminemia, who were receiving nutritional care. Serum concentrations of albumin, prealbumin and CRP were measured every week for 5 weeks. We defined patients whose serum albumin level was elevated by more than 0.2 g/dl after 5 weeks as those showing improved nutritional status. There was a significant difference in the prealbumin level between improved and unimproved patients at 5 weeks after the start of nutritional support. On the other hand, the prealbumin CRII value showed a significant difference between the groups at 1 and 2 weeks after the start of nutritional support. In conclusion, assessment of prealbumin CRII is useful for early prediction of improved nutritional status in patients with malnutrition

     民間のスポーツ施設に所属する小学生を対象に栄養サポートを行ったもののうち5例について、主に体格と摂取エネルギーとの関連について検討した。その結果、習い事としてスポーツ習慣のある小学生であっても、ほとんどの例で摂取エネルギーが不足傾向であった。小学校の中学年頃からは第二発育急進期に入り、身体が盛んに発育する時期であることに加えて、スポーツ習慣のある小学生ではその実施による消費エネルギーについても充足するエネルギーの摂取がまず重要である。そのための栄養サポートにおいては、定期的な体格のアセスメントと食事調査、推定エネルギー必要量の見直し等が必要であり、ベースラインからの変化が少なくとも2回以上は確認できる栄養サポートの仕組みが必要と考えられた

    Background and purposeThe Japanese Clinical Practice Guidelines for Management of Sepsis and Septic Shock 2016 (J-SSCG 2016), a Japanese-specific set of clinical practice guidelines for sepsis and septic shock created jointly by the Japanese Society of Intensive Care Medicine and the Japanese Association for Acute Medicine, was first released in February 2017 and published in the Journal of JSICM, [2017; Volume 24 (supplement 2)] https://doi.org/10.3918/jsicm.24S0001 and Journal of Japanese Association for Acute Medicine [2017; Volume 28, (supplement 1)] http://onlinelibrary.wiley.com/doi/10.1002/jja2.2017.28.issue-S1/issuetoc.This abridged English edition of the J-SSCG 2016 was produced with permission from the Japanese Association of Acute Medicine and the Japanese Society for Intensive Care Medicine.MethodsMembers of the Japanese Society of Intensive Care Medicine and the Japanese Association for Acute Medicine were selected and organized into 19 committee members and 52 working group members. The guidelines were prepared in accordance with the Medical Information Network Distribution Service (Minds) creation procedures. The Academic Guidelines Promotion Team was organized to oversee and provide academic support to the respective activities allocated to each Guideline Creation Team. To improve quality assurance and workflow transparency, a mutual peer review system was established, and discussions within each team were open to the public. Public comments were collected once after the initial formulation of a clinical question (CQ) and twice during the review of the final draft. Recommendations were determined to have been adopted after obtaining support from a two-thirds (> 66.6%) majority vote of each of the 19 committee members.ResultsA total of 87 CQs were selected among 19 clinical areas, including pediatric topics and several other important areas not covered in the first edition of the Japanese guidelines (J-SSCG 2012). The approval rate obtained through committee voting, in addition to ratings of the strengths of the recommendation, and its supporting evidence were also added to each recommendation statement. We conducted meta-analyses for 29 CQs. Thirty-seven CQs contained recommendations in the form of an expert consensus due to insufficient evidence. No recommendations were provided for five CQs.ConclusionsBased on the evidence gathered, we were able to formulate Japanese-specific clinical practice guidelines that are tailored to the Japanese context in a highly transparent manner. These guidelines can easily be used not only by specialists, but also by non-specialists, general clinicians, nurses, pharmacists, clinical engineers, and other healthcare professionals

    小児がんの医療に携わる医師と看護師を対象に、小児がんの疼痛緩和に関する医師・看護師の認識と課題を明らかにすることを目的に、幼児期と思春期のペーパーペイシェントを用いて質問紙調査を行った。その結果、除痛の方法に関して、疼痛の病態や原因を鑑別して患児の苦痛を軽減するために、医師・看護師は試行錯誤しながら治療やケアに取り組んでいる状況が明らかになった。幼児期は、医師は患児への説明に困難を感じており、検査や処置に関連した疼痛緩和の方法として鎮静下での実施を重視し、看護師は遊びやプレパレーション、家族の付添など心理的サポートを重視していた。思春期では、医師・看護師ともに疼痛評価や除痛の難しさを感じており、口内炎による疼痛について含嗽薬や強オピオイドなど薬物投与による予防的関わりを重視していた。相違点として、看護師は薬物投与以外に、クーリングや口腔ケア、食事の工夫を挙げていた。小児がんの疼痛緩和に向けて、他職種と連携を図り協働して取り組むことの重要性、小児がん治療の薬物選択の拡大、がん性疼痛治療ガイドラインの必要性が示唆された。A questionnaire survey was conducted involving physicians and nurses engaged in medical care of childhood cancer cases using paper patients concerning stages in early childhood and puberty, in order to clarify attitudes and issues of physicians and nurses towards pain‑relief in cases of childhood cancer. The results revealed that, regarding the administrative methods for pain‑relief, physicians and nurses applied treatment and care through trial and error in order to distinguish the cause of the clinical condition and relieving the children\u27s pain. In early childhood cases, physicians found it difficult to explain the matter to the children, and thus emphasized administering pain‑relief under sedation as a method associated with a test or procedure. Nurses, on the other hand, emphasized psychological support such as playing, preparation and family attendance. At puberty, both physicians and nurses found difficulty in evaluating the relieving of pain. They emphasized preventative involvement with drug administration such as mouthwashes or potent opioids for pain caused by stomatitis. Nurses also emphasized cooling, oral care, and dietary improvement. These results showed the importance of addressing pain‑relief while coordinating efforts and cooperating with other professionals, drug selection extension in the treatment of childhood cancer cases, and the necessity of guidelines for treating the cancer‑related pain

    The Japanese Clinical Practice Guidelines for Management of Sepsis and Septic Shock 2020 (J-SSCG 2020), a Japanese-specific set of clinical practice guidelines for sepsis and septic shock created as revised from J-SSCG 2016 jointly by the Japanese Society of Intensive Care Medicine and the Japanese Association for Acute Medicine, was first released in September 2020 and published in February 2021. An English-language version of these guidelines was created based on the contents of the original Japanese-language version. The purpose of this guideline is to assist medical staff in making appropriate decisions to improve the prognosis of patients undergoing treatment for sepsis and septic shock. We aimed to provide high-quality guidelines that are easy to use and understand for specialists, general clinicians, and multidisciplinary medical professionals. J-SSCG 2016 took up new subjects that were not present in SSCG 2016 (e.g., ICU-acquired weakness [ICU-AW], post-intensive care syndrome [PICS], and body temperature management). The J-SSCG 2020 covered a total of 22 areas with four additional new areas (patient- and family-centered care, sepsis treatment system, neuro-intensive treatment, and stress ulcers). A total of 118 important clinical issues (clinical questions, CQs) were extracted regardless of the presence or absence of evidence. These CQs also include those that have been given particular focus within Japan. This is a large-scale guideline covering multiple fields; thus, in addition to the 25 committee members, we had the participation and support of a total of 226 members who are professionals (physicians, nurses, physiotherapists, clinical engineers, and pharmacists) and medical workers with a history of sepsis or critical illness. The GRADE method was adopted for making recommendations, and the modified Delphi method was used to determine recommendations by voting from all committee members.As a result, 79 GRADE-based recommendations, 5 Good Practice Statements (GPS), 18 expert consensuses, 27 answers to background questions (BQs), and summaries of definitions and diagnosis of sepsis were created as responses to 118 CQs.
